Can US submarines and missile base commanders nowadays order their missiles to be launched if there is evidence that those governement and military folks with the authority of launching a full-sclae retaliation have just been blown to smithereens ??

Just sold out our allies in the Pacific Rim. Japan, Philippines, South Korea and the rest have just learned what most of Europe and Israel have had to learn. They have no friend in president quagmire.
So when North Korea does launch its rocket or rockets, then what? Will Beijing just sigh and roll their eyes? This administration is depending on the communist in China to reign in one of its "children" in order to ease tensions. NO WAY. The only reason the Chinese Army is on the border of North Korea is so to rush in and prop up the North Koran government. Nothing more and nothing less.
I would not be one bit surprised if this was orchestrated between NK and China. The Chinese know how to deal with crap like we have in the WH.
