Bexar County, Texas. I walked in and handed them my DL and voter registration card. Lots of signage saying ID REQUIRED and a.net list of what they would accept as ID.
Bristol Co., Mass. They ask address & name in that order, cross it off the rolls as they hand you a ballot. When you have finished with the ballot, you check out the same way before putting ballot in tabulator.
BTW, I was there at about 6pm and the place was pretty packed. A far cry from the primaries!
Sounds like New Mexico. If someone had gotten there before you, given your name and address, and voted, you'd be shit out of luck. "Sorry, sir, our records show you already voted."
Had to show ID in Cow Hampshire, and you can get a ballot without one, but you must sign an affidavit stating that your a legal resident.
Maryland - did not have to produce ID, but they asked my address and month/day of birth. I have no idea what would happen if I didn't know either answer. It was not to differentiate me from someone with a similar name because I know for a fact that no one else in my county has my last name.

Minnesota. No. Also I could have come back an hour later and registered on the spot as someone else with a bogus name and address and voted again. Between my voting the first time and re-registering again, I could have walked across the street (yes across the street) to a different precinct polling place and registered as someone else in that precinct and voted. COuld have traveled all over town all day long, and I'm sure there were buses full of people doing just that. What a joke.
None of it matters in Minn with the scum bag Sec of State that "counts" the votes. He is owned totally by the unions.
If I remember right, George Soros made a push a few years ago to install corrupt leftist Secretary of States across the nation and Minnesota's was his first success story. And that's how we ended up with Al Franken in the Senate.
Had to get a new drivers license when I moved to WA state, and they use that as a voter's registration at the Dept. of Licensing. Then I mailed in my ballot. First time voting in WA. I live about 8 miles from Cathy McMorris (amnesty supporter) Rogers home town. Had to hold my nose when I chose her over the Dem candidate. State initiative 594 (universal gun backround checks) NO! 591-(basically, leave our guns alone)-YES. I'm afraid, much like the rest of the left coast, Seattle and it's (Dem) freaks will continue to rule the day.
Hey, thanks for the state intiatives info. Those are always fun and interesting. Here in Michigan we had two - yes, two! - wolf hunting state resolutions. And here's the hilarious part: neither one of them matters! The Republican state legislature got so tired of fighting the liberal greenies who were trying to stop the wolf hunt, they finally passed a new law that assures a wolf hunt no matter what. (Michigan has gone from something like 3 wolves in the UP to something like 500 - the damn things are doing so well they're starting to make the UPers nervous. How do you send your kindergartner off to school when there's a wolf pack in the area?) I voted for the wolf hunt. Both times. Even if the vote doesn't matter.

The ranchers say Fish & Game won't do the right thing and it's gotten so bad that the local DA told ranchers she won't prosecute if they shoot the wolves. They've been killing a lot of sheep and some cows. Can the Government get anything right?
Sounds like California. The greentards outlawed hunting Mountain Lions years ago. Now they all wonder why they're seeing them in urban area backyards munching on FiFi for lunch. They outlawed using dogs for bear hunts, now every time you turn on the news there's a story about Yogi and Boo Boo breaking into someone's cabin.
Called overpopulation but they're clueless. Killing humans off to save Gaia is perfectly OK though.
When I was a kid camping in the UP with my parents, Dad drove us down to the local dump one night. It was like a drive-in movie. The place was lined with cars watching all the bears munch on garbage. It was fun. If a bear found something extra good, like some fried chicken bones, another bigger bear might try to take it away. Great fun (for the humans anyway). Then, at about the same time liberals managed to stop bear hunting, they also made the UP close its dumps. The result? Legions of suddenly voracious bears prowling around looking for a new easy food source, which of course meant they started attacking backpackers and campers all over the UP. For a few years, camping in the UP meant putting your life at risk. The most experienced backpackers I knew, a couple who had hiked throughout the west and in Alaska, got chased out of Porcupine Mountain state park by bears, leaving their backpacks and camping gear behind in their rush to escape. Liberals are so stupid. It doesn't really take much hunting before bears get the message that humans should be avoided. It's one of the big fables of the world that black bears are naturally shy about human contact. In fact, they will stalk humans and they are VERY good at stalking, trust me. They recently had a fatality in New Jersey for God's sake when the bear stalked and killed a Rutgers student: http://news.nationalpost.com/2014/09/24/black-bear-stalks-and-kills-new-jersey-student-the-first-fatal-bear-attack-in-the-state-in-150-years/. And now the UP has wolves AND bears to worry about. It's hard for me to believe I spent a week in the Porcupine Mountains as a kid, when I was in the Boy Scouts. Now, with the wolves and bears at much higher levels, I'm guessing the scoutmaster would consider a backpacking trip like that unsafe.

No ID required, I vote by mail. But, if memory serves they do check your driver's licence before they'll put you on the list to mail in. The People's Republic of Kalifornia.

In Union county, NC I had to sign a paper ballot which let me use the machine. No ID was required at all, but there were conspicuous signs posted that photo IDs would be required in 2016.
Durham County NC. Same thing.
Yes, unless the damn courts with their myriad of liberal jackass judges don't declare NC's new ID law unconstitutional like they've done with so many others. In Wisconsin the liberals sued and sued and sued, delaying the whole implementation of Voter ID until the Supreme Court finally ruled, "Hey, it's too late to implement this thing for the 2014 election." Never mind that the delays were caused by liberals, the SC ruled in their favor anyway.
