I think this is a cleaver fake, perhaps CGI.
He "racks" the first shot and fires, he racks it again ejecting the empty and loading another cartage, you can see the empty fall.
He fires his second shot and reloads, you can see the empty fall again.
Then he loads the third round and it misfires, when he "racks" the gun to re-cock it again the dud round should have been ejected.
That tells me the gun was empty. The chance of a dud round, a failure to extract and a hang fire all on the same round is just to much for me to believe.
I've seen this one before, the video skips before he blows his hat off. Definite Photoshop job.
I think you're right. In a really specific breakdown, at the normal speed, as he holds the gun and peers down the tube for a few frames nothing moves then like a snap the reflection changes and the discharge begins with the gun rotated slightly. I don't know exactly how they did it but the lack of any burn or stipple on either side of the hat baffles. There would certainly have to be some on the side that faced the discharge at such close range.
I prefer to watch my buddies actions with a weapon on a firing range before I ever go hunting with them. I tells volumes. Like others here I ALWAYS treat guns like a round is in the chamber and the hammer is cocked. If a weapon misfires, it needs to be unloaded, cleaned or taken to a gunsmith for repairs before another round is fired from it. Some folks have no common sense. This man is lucky. Im a lifetime member of the NRA also.
This is a classic example of why bleach needs to be added to the gene pool on occassions. I treat every firearm as if it is loaded. He knew that there was another round in the gun and is lucky he did not blow his head off.
Hmm, maybe staged? I kinda recall my NRA instructor saying something about this kind of situation. Honest to God folks, I'm NRA and firmly support the 2nd Amendment, but sometimes it scares the sh1t outta me to think that most places in the country any halfwit can go in and buy a gun off the rack with absolutely ZERO training or demonstrated competence in its use. Maybe I have a different POV living in the Massachusetts SSR, and not having grown up with firearms as a standard household accessory. But -in theory at least- I have no problem with requiring a basic safety course or at least a demonstration of knowledge before putting a firearm in someone's hands.
(I say "in theory" because I do know that usually any step like that is only going o be a first step toward increasingly burdensome regulation.

Ya know, when lefties start talking about "rights," we are usually there to speak of correlating "responsibilities." The right to bear arms is a spectacular right, and I do think we need to be careful of the responsibilities it entails.
Seemed like it to me too for a minute. Watched the blast again. It looked altogether real. I realize that's the idea when pulling a stunt but... I gotta go look at the hat again.
Well, well... No stippling on the hat. Ran vid through my editing prog and looked again. The audio doesn't fall out of sync until the last blast. The last blast does indeed look quite convincing. I know that the first thing you do when a misfire occurs is wait a moment for the delayed discharge. This guy seemed to get to looking down the spout pretty quick.
My guess is staged now. Maybe it will change again later. Triple guess.
That guy needs to take a class in weapon safety and go buy a lottery ticket.
